LABORATORY QUALITY GLOSSARY
read-and-acknowledge

Definition
Read-and-acknowledge is a documented process in which a staff member confirms receiving and reading specified information, commonly a new or revised controlled document. A useful record identifies the person, document, version, and date of acknowledgment. It can help track communication, especially when changes affect several shifts, sites, or professional groups with different access to routine meetings.
Acknowledgment is not the same as understanding, training, or demonstrated competence. The response to a document change should depend on its impact: a minor wording correction may need only notification, while a changed examination step can require explanation, supervised practice, assessment, and renewed authorization. Define who must acknowledge the change, the deadline, and how overdue responses are followed up before affected staff perform the changed activity independently.
Why it matters for accreditation
For accreditation, acknowledgment records help demonstrate controlled communication but should not be presented as sole evidence of competence. Assessors may ask staff to explain a recent change and compare their practice with the current procedure. Use risk-based implementation checks and appropriate assessment to verify that important revisions have been understood and incorporated into routine work.
